Instructions and Examples for Completing the Form

  1. Employer Information:

    Provide the legal name, organization name, and contact details of each employer involved.

    • Example:

      • Name of Employer: Michael Roberts

  2. Beneficiary Information:

    Enter the full legal name of the visa beneficiary as it appears on their official documents.

    • Example:

  3. Duration of Activities:

    List the specific start and end dates when the beneficiary will work for the employers. If dates are not available, leave this blank to indicate that work will start from visa approval and continue for up to three years.

    • Example:

      • Dates of Activities: From January 15, 2025, to January 15, 2028

      • If left blank, USCIS will assume activities commence upon visa approval and extend for three years.

  4. Description of Activities:

    Provide a concise description of the activities or roles the beneficiary will perform. Here are some examples:

    • Creative Direction and Production - Example: "Direct promotional video shoots, oversee event marketing campaigns, and coordinate with sponsors to enhance event visibility."

    • Consulting and Strategic Planning - Example: "Advise on brand strategy, develop marketing initiatives for international markets, and conduct performance reviews for marketing outcomes."

Submission and Distribution

Once completed, copies of this form will be automatically distributed to each employer and USCIS, ensuring all parties have the required documentation.
